medical power of attorney form for children when going out of country?

Hi, I am going out of the country with my husband. We are going on a cruise and my parents and his parents are splitting watching our kids for us for our anniversary. I know it will not always be easy to get ahold of us. Where do I find a form to give medical power of atty to our parents while we are out of town? Are there any free sites to print or do I need an atty?
