I am letting my son stay with my grandparents for about 3 months. Do I need to get a medical power of attorney

  2. you should type up a letter with all of his medical information and give consent for the grandparents to authorize medical treatment if needed while he is in their care and make sure you sign and date it. you should also include insurance info with that.
  3. You need to type up a letter giving your permission for the grandparents to seek medical care for the child while they are taking care of him. State your child's full name, date of birth and insurance information, any allergies, any pertinent medical information, meds the child may be on, along with your grandparents names, addresse and phone number. Give it a start date and an end date. Sign it and get it witnessed,at least, maybe even notarized. That way if anything happens, they can get him the care he needs, because unless it's a matter of life and death, he can't be treated without this if they can't get hold of you.

  5. DEFINATELY YES. A hospital or doctor has the right to refuse medical treatment without it.
